This RTI application is a concise format for an applicant seeking information from a State Public Information Officer under the Right to Information Act, 2005. It captures the applicant’s name, address, particulars of information required, subject matter, relevant period, and reference to a hearing or order where applicable. It also asks the PIO to provide details of the First Appellate Authority under Section 7 provisions, records whether information is to be collected in person, includes email and mobile contact details, BPL status, date and applicant signature.
